Event Details

The office will be accepting NEW applications only, verifying identifying documents, and collecting payment on applications. Please note that processing time is approximately 6-9 weeks for routine processing.

If more than one person in a family is applying, all applicants must appear in-person, additionally, please select the total number of people in the "ticket" section when making your reservation.

What to bring:

Completed passport application – printable form may be found HERE

2 Passport photos (2” x 2”) - Guide to Acceptable Photos may be found HERE

Payment: Checks or Money Orders (we cannot process debit or credit cards)– find complete list of fees HERE

$130 - Fee for First Time Adult Applicant

$100 - Fee for minor under 16 years old

Payments should be made out to: U.S. Department of State

In addition to: $35 Execution Fee for each applicant (Cash, Credit Card, or Money Order)

Execution Fee Payments should be made out to: Matteson Area Public Library District

Additional charges for expediting apply.

Please note that in addition to the above information, either all parents or guardians will need to be present, or one parent/guardian and a signed and notarized parental consent form, found HERE.
